Responsibilities
- Provide virtual patient assessments and care coordination via telehealth platforms
- Assist with remote medication reminders and health monitoring
- Document patient interactions accurately in electronic health records
- Collaborate with RNs and physicians to develop care plans
- Conduct periodic in-person patient observations and assistance at Phoenix-area facilities
- Respond to patient inquiries and provide emotional support
- Facilitate seamless transitions between virtual and in-person care
Qualifications
- Active Arizona CNA certification in good standing
- 1+ years of clinical experience in acute or long-term care
- Proficiency with telehealth technology and EHR systems
- Strong communication skills with ability to build rapport remotely
- Valid Arizona driver's license with reliable transportation
- Ability to work flexible hours including occasional weekends
- CPR/BLS certification from an accredited provider
- Pass background check and drug screening
