Responsibilities
- Conduct virtual patient assessments and care plan coordination
- Provide telehealth support and medication reminders via HIPAA-compliant platforms
- Facilitate virtual family education sessions on care protocols
- Collaborate with RNs and physicians on remote care adjustments
- Document patient interactions in EHR systems with precision
- Monitor patient vitals remotely through connected devices
- Respond to patient inquiries within 2-hour SLA
Qualifications
- Active Texas CNA certification (required)
- 1+ years clinical experience in home health or telehealth
- Proficiency with telehealth software (e.g., Doxy.me, Teladoc)
- Strong verbal/written communication skills
- Ability to multitask in virtual care environments
- Basic knowledge of EHR documentation systems
- CPR/BLS certification (preferred)
- High-speed internet and dedicated workspace
