Responsibilities
- Conduct virtual health assessments and medication reminders via telehealth platform
- Coordinate with RNs to create and update patient care plans remotely
- Provide compassionate telehealth support to patients and families
- Document patient interactions accurately in electronic health records (EHR)
- Collaborate with care teams to ensure continuity of virtual care services
- Monitor patient vitals and symptoms through remote monitoring devices
- Facilitate virtual wellness education sessions for patients and caregivers
Qualifications
- High school diploma or GED required
- Current Arizona CNA certification or willingness to obtain within 90 days
- Basic computer proficiency and reliable internet connection
- Strong communication and empathy skills
- Ability to pass background check and drug screening
- Experience with telehealth platforms preferred
- Valid Arizona driver's license (for occasional in-home visits)
- CPR/First Aid certification or willingness to obtain
