Responsibilities
- Provide remote telehealth support to patients under RN supervision
- Assist with virtual patient intake and health assessments
- Coordinate care plans and communicate patient needs to clinical teams
- Maintain accurate electronic health records (EHR) documentation
- Conduct virtual wellness checks and medication reminders

Qualifications
