Responsibilities
- Provide virtual patient assessments and health monitoring via telehealth platform
- Assist with medication management and care plan coordination
- Deliver emotional support and patient education remotely
- Maintain accurate electronic health records (EHR) documentation

Qualifications
- Active CNA certification in good standing
