Responsibilities
- Conduct virtual health assessments via telehealth platform
- Monitor patient vitals and medication adherence remotely
- Coordinate care plans with registered nurses and physicians
- Provide emotional support and health education to patients
- Document care activities in HIPAA-compliant electronic records
- Respond to emergency calls with rapid intervention protocols

Qualifications
- Active Texas CNA certification in good standing
- Minimum 1 year clinical experience (home health preferred)
