As a Remote CNA, you will play a pivotal role in patient triage and care coordination. You will utilize advanced telehealth platforms to support patients in New York and beyond.
Responsibilities
- Conduct virtual patient assessments and triage calls in accordance with NY state guidelines.
- Monitor patient vital signs remotely and report changes to the on-site medical team immediately.
- Document patient interactions accurately and securely using our HIPAA-compliant electronic health records (EHR) system.
- Coordinate care plans with local NY healthcare providers and family members.
- Provide health education and wellness support to patients via video conferencing and phone.
- Assist in the management of chronic disease cases through remote monitoring programs.
Qualifications
- Valid and active CNA (Certified Nursing Assistant) license in New York State.
- Previous experience in home health or telehealth preferred (but not mandatory for the right candidate).
- Strong computer literacy and proficiency with Microsoft Office Suite and EHR software.
- Reliable high-speed internet connection and a private, quiet workspace.
- Excellent communication skills and the ability to empathize with patients from diverse backgrounds.
- Ability to pass a background check and drug screening.
