Responsibilities
- Monitor Patient Health: Utilize remote monitoring devices and telehealth platforms to track vital signs and patient conditions in real-time.
- Virtual Care Coordination: Communicate with patients and their families via secure video conferencing to provide emotional support and education.
- Documentation: Accurately record patient data, symptoms, and progress notes in electronic health records (EHR) systems.
- Medication Management: Assist patients with medication reminders and remote adherence support as directed by the supervising nurse.
- Emergency Response: Respond promptly to urgent patient alerts and coordinate with on-call medical professionals when necessary.
- Health Education: Deliver virtual health education sessions to promote wellness and disease prevention.
- Team Collaboration: Collaborate effectively with nursing staff, physicians, and case managers to ensure comprehensive patient care.
Qualifications
- Certification: Current New York State Certified Nursing Assistant (CNA) license and certification.
- Experience: Minimum of 1-2 years of experience in a clinical or home health setting (preferred).
- Remote Proficiency: Demonstrated ability to work independently in a remote environment using video conferencing and telehealth software.
- Communication Skills: Excellent verbal and written communication skills, with a strong focus on empathy and patient-centered care.
- Computer Literacy: Proficiency in using electronic health records (EHR) and standard office software.
- CPR Certification: Current Basic Life Support (BLS) and/or CPR certification.
