Responsibilities
- Provide virtual patient monitoring and care coordination via telehealth platform
- Administer medications and document health updates remotely
- Coordinate with physicians, nurses, and family members for patient care plans
- Respond to patient inquiries and emergency calls during night shifts
- Conduct virtual wellness checks and health assessments
- Maintain accurate electronic health records (EHRs)
- Collaborate with care teams to ensure seamless patient transitions
Qualifications
- Active Maryland CNA certification
- Minimum 1 year clinical experience
- Night shift availability (11 PM - 7 AM)
- Proficiency with telehealth and EHR systems
- CPR/BLS certification

- Valid Maryland driver's license and reliable transportation
