Responsibilities
- Deliver virtual patient care via telehealth platforms including medication reminders and wellness checks
- Document patient interactions accurately in electronic health records (EHR) systems
- Coordinate with healthcare teams to ensure continuity of care
- Provide health education and emotional support to patients and families
- Monitor patient vitals and report abnormalities to registered nurses
- Assist with scheduling and appointment management
- Maintain strict HIPAA compliance in all communications
Qualifications
- High school diploma or GED equivalent
- Must be a CNA in good standing or willing to obtain NY certification
- Strong communication skills with empathy and patience
- Basic computer proficiency with ability to learn telehealth systems
- Ability to work independently with minimal supervision
- Valid NY driver's license (for occasional in-person visits)
