Responsibilities
- Coordinate patient care schedules and documentation between remote and in-person teams
- Assist with telehealth patient intake and follow-up communications
- Maintain accurate electronic health records (EHR) systems remotely
- Support CNA training initiatives through virtual modules and simulations
- Facilitate communication between patients, families, and clinical staff
- Monitor patient vitals and status updates via remote monitoring systems
- Participate in virtual interdisciplinary care conferences
Qualifications
