Responsibilities
- Deliver virtual patient assessments via telehealth platform
- Coordinate care plans with interdisciplinary teams
- Monitor patient vitals & medication adherence remotely
- Provide health education & emotional support virtually
- Document care notes in EHR systems with 99% accuracy
- Respond to patient inquiries within 2-hour SLA
- Collaborate with on-site staff for urgent cases
Qualifications
- Active Colorado CNA certification
- 1+ years telehealth or remote healthcare experience
- Proficient with EHR systems (Epic/Cerner preferred)
- Strong digital literacy & video conferencing skills
- Valid BLS/CPR certification
- Ability to pass background check & drug screen
- Weekend availability mandatory (Sat/Sun 8am-8pm)
- High-speed home internet required
