Responsibilities
- Conduct virtual health assessments via telehealth platform
- Assist patients with medication reminders and health tracking
- Provide emotional support and health education remotely
- Document patient interactions accurately in EMR system
- Coordinate with healthcare teams for patient care continuity
- Respond to urgent patient inquiries within SLA timeframes
- Maintain strict confidentiality of patient health information
Qualifications
- Active PA CNA certification in good standing
- Minimum 1 year clinical experience required
