Responsibilities
- Provide personalized in-home care and companionship to clients in Chula Vista
- Coordinate with families and medical teams via remote communication tools
- Assist with daily activities including meal preparation, medication reminders, and mobility support
- Monitor client health status and document care plans accurately
- Transport clients to appointments when required
- Report any concerns to care managers immediately
- Maintain strict confidentiality and HIPAA compliance
Qualifications
- Minimum 1 year caregiving experience or certification as Home Health Aide (HHA)
- Valid California driver's license and reliable vehicle
- Strong communication skills and ability to use remote tech tools
- Current CPR and First Aid certification
- Ability to pass background check and TB screening
- Compassionate demeanor with patience for elderly care
- Flexibility to work weekends and holidays as needed
