Responsibilities
- Provide virtual patient assessments and care coordination via telehealth platform
- Monitor and evaluate patient health status through digital health records
- Collaborate with interdisciplinary teams to develop patient care plans
- Deliver patient education and health coaching remotely
- Document clinical encounters accurately in EHR systems
- Respond to urgent patient inquiries within established SLAs
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Colorado (CO)
- Minimum 2 years of clinical nursing experience
- Telehealth or virtual care experience preferred
- BLS certification required
- Proficiency with EHR systems and telehealth platforms
- Strong communication and critical thinking skills
- Ability to work independently in remote setting
- Valid DEA license for controlled substance prescribing (if applicable)
