Responsibilities
- Provide comprehensive telehealth assessments and patient consultations via secure video platforms
- Develop and implement individualized care plans for diverse patient populations
- Collaborate with interdisciplinary teams to coordinate care transitions and interventions
- Document patient encounters accurately in EMR systems while maintaining HIPAA compliance
- Conduct remote medication reconciliation and patient education sessions
- Monitor patient progress and adjust care plans as needed based on clinical indicators
- Participate in quality improvement initiatives and clinical protocol development
Qualifications
- Active RN license in the state of Maryland (or compact license)
- Minimum 2 years of clinical nursing experience in acute care or specialty area
- Proficiency with telehealth platforms and EMR documentation systems
- Strong critical thinking skills and ability to assess patients remotely
- Excellent communication skills with demonstrated empathy and cultural competence
- Ability to work independently while collaborating effectively within a team
- Certification in Basic Life Support (BLS) and Telehealth Nursing preferred
- Flexibility to work varied shifts including evenings, weekends, and holidays
