Responsibilities
- Conduct virtual patient assessments and develop individualized care plans via telehealth platform
- Monitor and manage patient progress through electronic health records (EHR) systems
- Provide telehealth consultations for chronic disease management and acute care triage
- Collaborate with on-site teams to ensure seamless continuity of care
- Document clinical encounters accurately and maintain compliance with HIPAA regulations
- Participate in quality improvement initiatives and telehealth protocol development
Qualifications
- Active RN license in Oregon
- BSN degree (MSN preferred)
- Minimum 2 years of clinical nursing experience
- Proficiency in telehealth platforms and EHR systems
- Strong critical thinking and assessment skills
- Experience in case management or utilization review preferred
- Certification in telehealth nursing (e.g., C-TNBC) a plus
