Responsibilities
- Provide high-quality telehealth consultations and patient assessments
- Coordinate care plans with multidisciplinary teams
- Document patient encounters accurately in EHR systems
- Monitor chronic conditions and medication adherence
- Educate patients on health management strategies
- Respond to urgent patient inquiries via secure messaging
- Collaborate with case managers for complex patients
Qualifications
- Active AZ RN license (compact license accepted)
- 2+ years clinical nursing experience
- BLS certification required
- Telehealth or virtual care experience preferred
- Strong computer literacy and tech-savvy
- Excellent communication and critical thinking skills
- Ability to work independently in remote setting
