Responsibilities
- Provide comprehensive telehealth assessments and patient consultations via secure virtual platforms
- Collaborate with multidisciplinary teams to develop and implement patient care plans
- Monitor and evaluate patient progress, adjusting interventions as needed
- Document clinical encounters accurately in electronic health records (EHR)
- Educate patients on chronic disease management and medication adherence
- Participate in quality improvement initiatives and peer review processes
- Respond to urgent patient inquiries during designated on-call hours
Qualifications
- Active RN license in Arizona (AZ RN license required)
- Minimum 2 years of clinical nursing experience
- Telehealth or virtual care experience preferred
- BLS certification required; ACLS preferred
- Strong assessment skills and critical thinking abilities
- Excellent communication skills with proficiency in virtual platforms
- Ability to work independently with minimal supervision
- Experience with EHR systems (e.g., Epic, Cerner)
