Responsibilities
- Provide high-quality virtual patient assessments and care coordination via telehealth platform
- Collaborate with interdisciplinary teams to develop and implement personalized care plans
- Monitor patient progress and adjust interventions based on clinical data
- Document patient encounters accurately in electronic health records (EHR)
- Educate patients and families on health management and treatment protocols

Qualifications
- Active Washington State RN license (compact license accepted)
- Bachelor of Science in Nursing (BSN) preferred
- Minimum 2 years of acute care nursing experience
