Responsibilities
- Deliver virtual patient assessments and consultations via telehealth platform
- Monitor patient health data remotely and intervene when necessary
- Develop and implement individualized care plans in collaboration with clinical teams
- Coordinate patient transitions between care settings
- Document patient encounters accurately in EMR systems
- Educate patients on self-management and treatment protocols
- Participate in interdisciplinary case reviews
Qualifications
- Active RN license in District of Columbia
- Minimum 2 years of clinical nursing experience
- BLS and ACLS certifications required
- Telehealth or virtual care experience preferred
- Strong proficiency with EMR and telehealth platforms
- Exceptional critical thinking and communication skills
- Ability to work independently in a remote setting
- Bachelor's degree in Nursing (BSN) preferred
