Responsibilities
- Conduct remote patient triage and health assessments via secure phone and video calls.
- Monitor and document patient vital signs and symptoms accurately in EHR systems.
- Provide education and support to patients regarding chronic disease management and medication adherence.
- Collaborate with physicians and care coordination teams to ensure continuity of care.
- Escalate critical patient issues to on-site medical staff immediately.
Qualifications
- Current Registered Nurse (RN) license issued by the State of Tennessee.
- Graduation from an accredited Nursing School (New Grads welcome).
- Strong computer literacy and typing proficiency.
- Excellent verbal and written communication skills.
- Ability to maintain patient confidentiality and professionalism.
