Responsibilities
- Deliver high-quality telehealth consultations and patient assessments via secure virtual platforms
- Coordinate patient care plans with interdisciplinary teams and community resources
- Monitor chronic conditions and provide remote medication management guidance
- Document clinical encounters accurately in electronic health records (EHR) systems
- Participate in quality improvement initiatives and telehealth protocol development
- Provide health education and wellness coaching to patients and families
- Respond to urgent patient inquiries during designated on-call hours
Qualifications
- Active Registered Nurse license in District of Columbia (or compact state)
- Minimum 2 years of clinical nursing experience in acute care or telehealth
- BLS certification and current DEA registration for controlled substance prescribing
- Proficiency with telemedicine platforms and EHR systems (Epic preferred)
- Strong assessment skills and ability to make clinical decisions remotely
- Excellent communication skills with ability to build rapport virtually
- Experience with chronic disease management and health coaching
- Masters in Nursing (MSN) or Telehealth certification preferred
