Responsibilities
- Deliver virtual patient assessments and care coordination
- Monitor and interpret patient data via telehealth platforms
- Collaborate with interdisciplinary care teams
- Document patient encounters accurately in EMR systems
- Educate patients on health management and medication adherence

Qualifications
- Active California RN license (or eligible)
