Responsibilities
- Conduct comprehensive telephone/telehealth patient assessments using evidence-based protocols
- Provide clinical guidance to patients for acute/chronic conditions and medication inquiries
- Collaborate with providers to coordinate patient care transitions and follow-ups
- Document encounters accurately in Epic EMR systems
- Educate patients on home care management and preventive services
- Triage emergent cases according to EMTALA compliance standards
- Participate in quality improvement initiatives and protocol development
Qualifications
- Active RN license in Minnesota (or compact state)
- Minimum 3 years ER/urgent care or telehealth experience
- BLS/ACLS certification required
- Proficient in Epic EMR and telehealth platforms
- Exceptional critical thinking and communication skills
- Ability to work independently with minimal supervision
- Experience with triage protocols (e.g., ESI, Manchester)
- Strong computer literacy and reliable home internet
