Responsibilities
- Conduct comprehensive telehealth assessments to determine patient acuity and prioritize care needs
- Provide evidence-based clinical guidance and treatment recommendations via phone/video consultations
- Collaborate with physicians and specialists to coordinate patient care plans and referrals
- Maintain accurate electronic health records (EHR) documentation in compliance with HIPAA regulations
- Monitor patient outcomes and follow up on critical cases to ensure resolution
- Educate patients on self-care management and preventive health strategies
- Participate in continuous quality improvement initiatives for triage protocols
Qualifications
- Active RN license in New York State with minimum 2 years clinical experience
- BLS/ACLS certification and current BCLS renewal
- Proven expertise in emergency department or urgent care triage
- Exceptional critical thinking and decision-making skills
- Strong proficiency with telehealth platforms and EHR systems
- Excellent communication abilities with diverse patient populations
- Ability to work independently while collaborating effectively in virtual teams
- Experience with remote nursing workflows preferred
