Responsibilities
- Conduct comprehensive phone and video triage assessments for patients presenting with acute and chronic symptoms.
- Apply clinical reasoning to determine the urgency of patient needs and initiate appropriate care plans.
- Educate patients on self-care management, medication usage, and preventative health measures.
- Document all patient interactions accurately and timely in the Electronic Health Record (EHR).
- Collaborate with physicians and other care team members to ensure seamless continuity of care.
- Identify and escalate critical cases or red-flag symptoms immediately according to protocol.
- Stay updated on current clinical guidelines and triage best practices.
Qualifications
- Current and active RN license in the State of Tennessee.
- Minimum of 2 years of nursing experience, with a preference for triage or telehealth experience.
- Certification in Emergency Nursing (CEN) or Telehealth Nursing preferred.
- Strong computer proficiency and ability to navigate EHR systems remotely.
- Excellent communication skills, with the ability to build rapport over the phone/video.
- Ability to remain calm under pressure and make sound medical judgments quickly.
- Reliable high-speed internet and a dedicated home office space.
