We are looking for a visionary Triage Nurse who excels in fast-paced environments and is committed to patient safety.
Responsibilities
- Conduct comprehensive patient assessments via phone and secure patient portal to determine appropriate level of care and urgency.
- Make real-time clinical decisions regarding patient disposition, including hospital admission, emergency department transport, or home care instructions.
- Collaborate with physicians, nurse practitioners, and care coordinators to ensure continuity of care and optimal patient outcomes.
- Educate patients and families on disease management, medication adherence, and preventative health strategies.
- Maintain accurate and detailed electronic health records (EHR) in compliance with HIPAA and organizational policies.
- Participate in quality improvement initiatives and continuous education programs to stay current with clinical best practices.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the State of Hawaii.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Minimum of 3-5 years of critical care or emergency department nursing experience preferred.
- Strong proficiency in telehealth triage software and EMR systems.
- Excellent verbal and written communication skills with a focus on patient empathy and clarity.
- Ability to work independently and make critical decisions under pressure.
