Responsibilities
- Deliver high-quality patient assessments and care coordination via telehealth platforms
- Monitor and interpret patient vitals from remote monitoring systems
- Collaborate with on-site teams to develop personalized care plans
- Document patient interactions accurately in EHR systems
- Provide patient education and health coaching through virtual channels
- Participate in interdisciplinary case reviews and quality improvement initiatives
- Respond to emergency alerts and coordinate rapid response protocols
Qualifications
- Active RN license in Massachusetts
- BSN degree (MSN preferred)
- 3+ years clinical nursing experience in acute care
- Telehealth or remote care certification
- Proficiency with EHR systems (Epic preferred)
- Strong critical thinking and decision-making skills
- Exceptional communication and empathy
- Ability to work independently with minimal supervision
