Responsibilities
- Patient Assessment: Conduct thorough telephone and in-person assessments to evaluate patient symptoms, medical history, and acuity levels.
- Triage Prioritization: Utilize clinical judgment to prioritize patient needs and determine the appropriate level of care (e.g., home care, urgent care, emergency department).
- Provider Collaboration: Communicate findings clearly to physicians and nurse practitioners to facilitate timely treatment plans.
- Documentation: Accurately document patient interactions, triage decisions, and follow-up instructions in the Electronic Health Record (EHR).
- Health Education: Provide patients and caregivers with clear instructions on medication management, home remedies, and when to seek further medical attention.
- Call Management: Manage a high-volume queue of incoming calls efficiently while maintaining a calm and empathetic demeanor.
- Quality Assurance: Participate in quality improvement initiatives and adhere to all hospital protocols and regulatory standards.
Qualifications
- License: Active and unencumbered Registered Nurse (RN) license in the State of California.
- Certifications: Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required.
- Experience: Minimum of 2-3 years of clinical nursing experience, with specific experience in triage or call center nursing preferred.
- Communication: Excellent verbal communication skills with the ability to de-escalate tense situations and explain complex medical concepts clearly.
- Technical Skills: Proficiency in using Electronic Health Records (EHR) systems (e.g., Epic, Cerner) and computer applications.
- Problem Solving: Strong critical thinking and decision-making skills under pressure.
- Emotional Intelligence: High level of empathy and patience when dealing with distressed patients.
