Responsibilities
- Conduct comprehensive telephone and video triage assessments to determine appropriate level of care.
- Evaluate patient symptoms, medical history, and current status to recommend appropriate treatment plans.
- Coordinate patient referrals to specialists, urgent care, or emergency services when necessary.
- Provide patient education and self-care instructions to promote recovery.
- Maintain accurate and timely documentation of all triage encounters.
- Collaborate with physicians and other healthcare providers to ensure continuity of care.
Qualifications
- Current and valid Registered Nurse (RN) license in the state of Texas.
- BSN preferred; Bachelor's degree in Nursing is a plus.
- Minimum of 2 years of clinical nursing experience, preferably in a triage or call center setting.
- Strong communication and active listening skills.
- Ability to remain calm and make critical decisions under pressure.
- Proficiency with electronic health records (EHR) systems.
