Job Summary:
As a Triage Nurse, you will be the first point of contact for patients seeking medical advice. You will utilize your clinical expertise to assess symptoms, provide appropriate guidance, and facilitate timely referrals to providers when necessary.
Responsibilities
- Assess patient symptoms via phone or secure messaging to determine the appropriate level of care and urgency.
- Provide evidence-based nursing advice, including home care instructions and medication management.
- Document all interactions accurately and comprehensively within the Electronic Health Record (EHR).
- Collaborate effectively with physicians, advanced practice providers, and other care team members to ensure continuity of care.
- Screen and triage inbound calls efficiently, managing high call volumes while maintaining quality standards.
- Identify potential health risks and escalate critical cases immediately to the on-call provider.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Minnesota.
- Minimum of 2 years of experience in a clinical setting, preferably in a triage or urgent care environment.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications are required.
- Strong communication skills with the ability to de-escalate patient concerns and convey complex medical information clearly.
- Proficiency with Electronic Health Records (EHR) systems (e.g., Epic, Cerner, Athenahealth).
- Ability to work independently while maintaining a high level of situational awareness.
