Responsibilities
- Conduct Comprehensive Assessments: Evaluate patient symptoms via phone or secure messaging to determine the appropriate level of care and urgency.
- Route Care Efficiently: Coordinate patient flow to emergency departments, urgent care centers, or primary care physicians as needed.
- Facilitate Patient Education: Provide clear, actionable health advice and medication management instructions to patients and caregivers.
- Document Clinical Data: Accurately record patient interactions, triage decisions, and care plans in our Electronic Health Record (EHR) system.
- Collaborate with Providers: Communicate critical findings to on-call physicians and care managers to ensure continuity of care.
- Support Clinical Protocols: Adhere strictly to evidence-based clinical guidelines and company policies during every interaction.
Qualifications
- Active RN License: Valid Registered Nurse license issued by the state of Pennsylvania (or willingness to relocate).
- Certifications: Current BLS and ACLS certifications required.
- Clinical Experience: Minimum of 2 years of nursing experience in a hospital or clinical setting preferred.
- Communication Skills: Excellent verbal and written communication skills with a focus on patient empathy and clarity.
- Problem Solving: Strong critical thinking and decision-making abilities under pressure.
- Technical Proficiency: Comfortable navigating computer systems and telehealth platforms.
- Training Attitude: Demonstrated willingness to participate in and complete the provided paid training curriculum.
