Responsibilities
- Provide compassionate in-home care to elderly and disabled patients in NYC boroughs
- Assist with daily living activities including bathing, dressing, and medication administration
- Monitor vital signs and document patient progress via our secure telehealth platform
- Coordinate with RNs and care teams through virtual case management sessions
- Transport patients to medical appointments using company-provided vehicles
- Implement personalized care plans under remote supervision
- Maintain HIPAA-compliant electronic health records
Qualifications
- Active NY State CNA certification in good standing
- Minimum 1 year experience in home care or hospital setting
- BLS/CPR certification (or willingness to obtain)
- Reliable transportation and clean driving record
- Proficiency with telehealth technology (training provided)
- Ability to lift 50 lbs and stand for extended periods
- Clear criminal background check and drug screening
- Spanish bilingual preferred but not required
