Responsibilities
- Conduct virtual patient assessments via telehealth platforms
- Coordinate care plans with registered nurses and physicians
- Monitor patient vitals and medication adherence remotely
- Provide health education and emotional support virtually
- Maintain accurate electronic health records (EHR)
- Respond to patient inquiries via secure messaging systems
- Collaborate with interdisciplinary care teams
Qualifications
- Active Arizona CNA certification
- Minimum 1 year clinical experience
- Proficient with telehealth technology (training provided)
- Strong communication and empathy skills
- Ability to work independently in a remote setting
- Valid CPR certification
- Reliable high-speed internet connection
- Background check and drug screen clearance
