Responsibilities
- Provide compassionate virtual patient assessments and care coordination
- Monitor and interpret patient data using telehealth platforms
- Collaborate with multidisciplinary teams to develop personalized care plans
- Document patient encounters accurately in electronic health records
- Educate patients on chronic disease management and preventive care
- Respond to patient inquiries and triage urgent situations remotely

Qualifications
- Valid RN license in the state of Missouri
- Associate or Bachelor's degree in Nursing (ADN/BSN)
- Basic Life Support (BLS) certification
