Responsibilities
- Provide comprehensive telehealth nursing assessments and care coordination for diverse patient populations
- Monitor and interpret patient data through electronic health records (EHR) and remote monitoring systems
- Collaborate with multidisciplinary teams to develop and implement patient care plans
- Deliver patient education on chronic disease management and medication adherence
- Respond to emergency situations and triage critical patient calls
- Document care activities accurately and maintain strict HIPAA compliance
- Participate in continuous quality improvement initiatives
Qualifications
- Active California RN license (required)
- BSN degree with 3+ years of clinical nursing experience
- Telehealth or remote nursing experience preferred
- BLS and ACLS certifications (current)
- Proficiency in EHR systems and telehealth technology
- Strong critical thinking and decision-making skills
- Excellent communication and interpersonal abilities
- Ability to work independently with minimal supervision
