Responsibilities
- Provide virtual patient assessments and health monitoring via telehealth platforms
- Assist patients with medication reminders and health education remotely
- Coordinate with healthcare teams for seamless care continuity
- Maintain accurate electronic health records (EHR) documentation
- Deliver compassionate patient support through video consultations
- Respond to patient inquiries and triage urgent concerns
- Collaborate with nurses and physicians for care plan adjustments
Qualifications
- Active California CNA license in good standing
- Minimum 1 year clinical experience in geriatric or home care
- Proficient with telehealth technology (Zoom, Epic, etc.)
- Strong communication and empathy skills
- CPR/BLS certification (current)
- Ability to work independently in a remote setting
- Flexible availability including evenings/weekends
- Background check and TB clearance required
