Responsibilities
- Provide comprehensive remote patient assessments via telehealth platforms
- Monitor vital signs and health metrics using advanced digital monitoring tools
- Collaborate with in-hospital teams to coordinate patient care plans
- Document clinical observations accurately in EMR systems
- Educate patients on medication management and chronic disease prevention
- Respond to urgent patient alerts within 15-minute SLA
- Participate in interdisciplinary case reviews and quality improvement initiatives
Qualifications
- Active Florida RN license (unrestricted)
- Minimum 2 years acute care nursing experience
- BLS and ACLS certifications required
- Proficiency with telehealth technologies (e.g., Epic MyChart, Teladoc)
- Strong critical thinking and decision-making abilities
- Excellent written and verbal communication skills
- Ability to work independently with minimal supervision
- Experience with remote patient monitoring systems preferred
