Responsibilities
- Conduct virtual patient assessments and health evaluations
- Develop and implement individualized care plans
- Provide telehealth consultations and patient education
- Coordinate care with multidisciplinary teams
- Document clinical notes accurately in EMR systems
- Monitor patient outcomes and adjust interventions
- Participate in quality improvement initiatives
Qualifications
- Active RN license in Ohio (or compact state)
- Minimum 2 years clinical nursing experience
- Telehealth or virtual care experience preferred
- Proficiency with EMR and telehealth platforms
- Strong critical thinking and assessment skills
- Excellent communication and documentation abilities
- BLS certification required
