Responsibilities
- Provide high-quality remote patient assessments and care coordination via telehealth platform
- Monitor and evaluate patient vitals, symptoms, and treatment adherence
- Collaborate with multidisciplinary teams to develop personalized care plans
- Document patient encounters accurately in EMR systems
- Educate patients on chronic disease management and medication protocols
- Respond to urgent patient inquiries and escalate critical cases appropriately
- Participate in continuous quality improvement initiatives
Qualifications
- Active AZ RN license in good standing
- Minimum 2 years of clinical nursing experience
- Telehealth or virtual care experience preferred
- BLS/ACLS certification required
- Strong proficiency with telehealth technology and EMR systems
- Exceptional communication and critical thinking skills
- Ability to work independently with minimal supervision
