Responsibilities
- Perform rapid and accurate patient assessments via phone and in-person to determine the urgency of medical needs.
- Apply clinical protocols and critical thinking skills to triage patients into appropriate care pathways.
- Document patient interactions and medical histories accurately in the Electronic Health Record (EHR).
- Collaborate with physicians and other healthcare professionals to ensure seamless patient transitions.
- Provide health education and guidance to patients regarding chronic disease management and wellness.
- Manage high call volumes while maintaining high-quality communication standards.
Qualifications
- Current and valid RN (Registered Nurse) license in the State of New Jersey.
- Minimum of 2 years of experience in a clinical setting, preferably in triage or emergency medicine.
- Certifications in BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) required.
- Strong computer proficiency with Electronic Health Record (EHR) systems.
- Excellent verbal and written communication skills.
- Ability to remain calm under pressure and make sound decisions in fast-paced environments.
