Responsibilities
- Deliver personalized patient care including bathing, dressing, and feeding assistance
- Monitor and document vital signs, patient responses, and health changes
- Assist with mobility transfers, ambulation, and rehabilitation exercises
- Provide emotional support and companionship to patients and families
- Maintain clean, safe patient environments and infection control protocols
- Collaborate with RNs and healthcare team on care plans
- Administer medications under RN supervision as per state guidelines
Qualifications
- Active DC CNA certification required
- Minimum 1 year long-term care or acute care experience
- BLS/CPR certification (or willingness to obtain within 30 days)
- Proficiency in electronic health records (EHR) systems
- Strong communication and interpersonal skills
- Ability to lift up to 50 pounds and stand/walk for extended periods
- Background check and drug screening clearance
- Valid driver's license and reliable transportation
