Responsibilities
- Provide direct patient care, assisting with daily activities such as bathing, dressing, and grooming.
- Monitor and record vital signs, including blood pressure, temperature, and pulse, reporting any abnormalities to the RN.
- Assist patients with mobility, transfers, and ambulation using proper body mechanics and assistive devices.
- Prepare and serve meals, ensuring dietary restrictions and feeding requirements are met.
- Maintain a clean, safe, and therapeutic environment for patients by following sanitation and infection control protocols.
- Document patient care plans, progress notes, and incidents accurately in electronic health records.
- Communicate effectively with patients, families, and the interdisciplinary healthcare team.
Qualifications
- Current and active Ohio CNA license required.
- Basic Life Support (BLS) certification through the American Heart Association.
- High school diploma or GED required.
- Previous experience in a hospital, long-term care, or home health setting is preferred.
- Strong interpersonal and communication skills with a focus on patient-centered care.
- Ability to stand for long periods and perform physical tasks, including lifting and assisting patients.
