Responsibilities
- Provide daily personal care assistance including bathing, dressing, and grooming for elderly and disabled patients
- Monitor vital signs and report changes in patient condition to nursing staff
- Assist with mobility, positioning, and transfer activities using proper techniques
- Document patient care accurately and maintain electronic health records
- Support meal preparation and feeding assistance when required
- Collaborate with interdisciplinary care teams to develop and implement care plans
- Maintain a clean, safe, and organized patient environment
Qualifications
- Active New York State CNA certification in good standing
- Minimum 1 year of recent long-term care or acute care experience
- CPR/BLS certification from American Heart Association
- Ability to lift up to 50 pounds and stand for extended periods
- Strong communication skills and compassionate patient interaction approach
- Proficiency with electronic health record systems (e.g., Epic, Cerner)
- Valid NYC Health Department clearance and TB screening
