Responsibilities

  • Provide direct patient care and assistance with Activities of Daily Living (ADLs) such as bathing, dressing, and grooming.
  • Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration rates.
  • Assist patients with mobility, transfers, and ambulation using proper body mechanics and assistive devices.
  • Administer medication as directed by the Registered Nurse or Licensed Practical Nurse, following strict safety protocols.
  • Maintain a clean, safe, and sanitary patient environment to prevent infection and ensure patient comfort.
  • Document patient care activities, changes in condition, and responses to treatment in electronic medical records accurately.
  • Communicate effectively with patients, families, and the interdisciplinary healthcare team to ensure holistic care.

Qualifications

  • Current and active State of Wisconsin CNA Certification.
  • Current Basic Life Support (BLS) certification for Healthcare Providers.
  • High School Diploma or General Education Development (GED) certificate.
  • Previous experience in a long-term care facility, hospital, or home health setting is preferred but not mandatory.
  • Strong interpersonal and communication skills with a patient-centered approach.
  • Ability to stand for extended periods and lift up to 50 lbs as needed.
