Responsibilities
- Provide remote patient assessments and care coordination via telehealth platform
- Assist patients with medication management and health monitoring
- Collaborate with interdisciplinary care teams to develop patient plans
- Deliver compassionate virtual support to geriatric and chronic care patients
- Maintain accurate electronic health records (EHR) documentation
- Conduct virtual wellness checks and emergency response coordination
- Facilitate patient education on care plans and community resources
Qualifications
- Active Virginia CNA certification in good standing
- Minimum 2 years clinical experience preferred
- Proficiency with telehealth platforms and EHR systems
- Strong communication and empathy skills
- Ability to work independently in remote environment
- Basic computer literacy and reliable internet connection
- CPR/BLS certification (or willingness to obtain)
