Responsibilities
- Coordinate patient care plans via telehealth platforms
- Conduct virtual health assessments and medication reminders
- Document patient progress in EMR systems
- Collaborate with RNs and care teams remotely
- Schedule and coordinate in-home patient visits (as needed)
- Provide emotional support to patients and families
- Participate in virtual care conferences
Qualifications
- Active Pennsylvania CNA certification
- 1+ years of clinical experience
- Proficient with telehealth technology
- Strong communication and documentation skills
- Ability to work independently with minimal supervision
- Valid Pennsylvania driver's license (for occasional visits)
- CPR/BLS certification
- Comfortable with remote work technology
