Responsibilities
- Coordinate patient care plans via telehealth platforms
- Monitor vital signs and health metrics remotely using digital tools
- Communicate patient updates to healthcare providers and family members
- Assist with medication management reminders via virtual systems
- Document care activities in electronic health records (EHR)
- Conduct virtual wellness assessments and patient education
- Collaborate with interdisciplinary care teams during remote consultations
Qualifications
- Active Pennsylvania CNA certification required
- 1+ years of clinical experience in home care or hospital setting
- Proficiency with telehealth software and EHR systems
- Strong written and verbal communication skills
- CPR/BLS certification (current)
- Ability to travel locally for patient visits (mileage reimbursed)
- Pass background check and drug screening
