Responsibilities
- Provide direct patient care, including bathing, dressing, and grooming assistance, to ensure residents' comfort and dignity.
- Monitor and record vital signs (blood pressure, pulse, respiration, temperature) and report any significant changes to the RN or charge nurse immediately.
- Assist with mobility, transfers, and positioning to prevent falls and promote patient safety.
- Maintain a clean, safe, and organized patient environment, adhering to all infection control protocols.
- Document patient care activities, behaviors, and changes accurately in electronic health records.
- Communicate effectively with residents, families, and interdisciplinary team members to ensure holistic care.
Qualifications
- Current and active Oregon CNA Certification (State License # required).
- Basic Life Support (BLS) certification from the American Heart Association.
- High school diploma or GED equivalent.
- Strong interpersonal and communication skills with a patient, empathetic demeanor.
- Ability to stand, walk, and lift up to 25 lbs frequently throughout the shift.
- Valid driver's license and reliable transportation (optional but preferred).
